Please note, if you have more than one child, you will need to submit a separate request form for each child.

Please provide as much detail as possible, including the reason this absence cannot occur outside of the school term time as gazetted by the Government of Western Australia. As school-age students are mandated to attend school during term time, the information will be provided to the Department of Education, when required. Please note, only stating ‘Family Holiday’ is not an acceptable reason and the absence will not be approved.

a.      I acknowledge that my child will be responsible for contacting his/her teacher regarding any work or assessments that will be missed during this time.

b.      I acknowledge that it will be my child’s responsibility to stay up-to-date with any content covered during this time.

c.      I acknowledge that the Principal / Deputy Principal Teaching and Learning reserve the right to determine whether this absence is ‘approved’ or ‘unapproved’.

d.      I acknowledge that if deemed ‘approved’, it will be up to the discretion of the classroom teacher to determine which of the three modifications occur to any assessments missed, in accordance with the College Assessment Policy..

e.      I acknowledge that if deemed ‘unapproved’, my child will need to sit any in-class missed assessment immediately upon their return to school within the first three Missed Assessment sessions that occur, and that failure to do so will result in a score of ‘zero’ for this task.

f.       I acknowledge that if deemed ‘unapproved’, my child will need to submit any out-of-class assessment to the classroom teacher on or before the assessment due date (electronic submission is acceptable). Failure to do so will result in penalty in accordance with our College Assessment Policy.

g.      I acknowledge that an extended or multiple absences may affect my child’s capacity to demonstrate learning or understanding of curriculum points and as such, could impact final grading and/or may have implications for achievement of WACE (if in Year 11 or 12).
